Memory Care Coordinator (LPN/RN)

Summary:
The Memory Care Coordinator is responsible for the overall oversight of the memory care unit.
The Coordinator will ensure all proper programming is occuring on a daily basis.
ESSENTIAL DUTIES AND
Responsibilities:
Complete MDS and other needed assessments Manage staff and day to day operations on the memory care unit Run care plan meetings for residents on the memory care unit Assist with daily programming when needed Work with other department leaders as needed to ensure a smooth operation on the memory care unit Educate staff on the Memory Care Unit related to Dementia and programing Ensure daily programming is taking place All other duties as assigned
Qualifications:
RN/LPN Required Certified Dementia Practitoner preferred Long Term Care Experiance preferred Recommended Skills Assessments Caring For People With Dementia Long Term Care Computer Programming Estimated Salary: $20 to $28 per hour based on qualifications.
